Ratings reflect the CMS Health Inspection rating on a 1-5 scale. "Critical flags" combine CMS abuse citations, immediate-jeopardy findings, and repeat serious deficiencies. Medication percentages are facility-level averages of CMS-reported long-stay resident measures. Data pulled from the live Palmelle directory on 2026-10-22; refreshed daily.
